ACYCC Initial Questionnaire
The following PDF document is a questionnaire which, once completed, provides important background, history and context details.  Being able to consider together some of the areas contained in this questionnaire will allow us to narrow-in on specific areas of focus or issue.  Some questions will be relevant in a young person's life, and some won't apply in certain cases...these are simply crossed-off.
